Oral Wegovy: the daily pill for obesity treatment
Oral Wegovy, better known as the Wegovy pill, is a daily tablet of semaglutide at a dose of 25 mg. It is the first GLP-1 medication approved in pill form for the treatment of obesity. What’s more, it has shown the highest effectiveness seen so far for an oral obesity treatment. Patients who stayed on treatment lost about 17% of their body weight on average, and one in three lost 20% or more.
In this guide, you will learn what the pill does, who it is right for, how to take it correctly and what is known about its safety. You will also find a comparison with injections and other pills, and an overview of the situation in Israel.
What is oral Wegovy?
The active ingredient is the same as in Wegovy and Ozempic injections. The difference lies in how it is delivered. To help the protein survive the stomach, the tablet contains an absorption enhancer called SNAC. It protects the medication and allows it to pass into the bloodstream through the stomach wall.
Only a small fraction of each dose is absorbed. That is why the tablet dose is much higher than the injected dose: 25 mg a day, compared with 2.4 mg a week by injection. Even so, blood levels are similar.
How it differs from Rybelsus
Rybelsus is also semaglutide in pill form. However, it is approved only for type 2 diabetes, and at much lower doses. The Wegovy pill, by contrast, is intended for the treatment of obesity.
How oral Wegovy works in the body
Semaglutide mimics GLP-1, a hormone the gut releases after a meal. The hormone acts in several places at once:
- In the brain: it increases fullness, reduces hunger and quiets “food noise.”
- In the stomach: it slows emptying, so the feeling of fullness lasts longer.
- In the pancreas: it boosts insulin release when blood sugar is high and lowers glucagon.
As a result, people eat less without a constant battle against hunger. You will find a more detailed explanation in my article on GLP-1 medications and the brain.
Who is the treatment for?
According to the US prescribing information, the medication is intended for adults with obesity. It is also suitable for people with overweight who have at least one weight-related condition, such as high blood pressure, dyslipidemia or sleep apnea. In addition, it is approved to reduce the risk of cardiovascular events in patients with overweight and established heart disease. This indication is based on the SELECT trial of injectable semaglutide.
Treatment is always given alongside tailored nutrition and physical activity. A doctor determines whether it is right for each person after a full medical evaluation.
What the OASIS 4 trial showed
The approval was based mainly on the OASIS 4 trial, published in 2025 in the New England Journal of Medicine. The trial included 307 adults with obesity or overweight, without diabetes. Two-thirds received oral Wegovy at 25 mg, and one-third received placebo. All participants also received lifestyle support, over 64 weeks.
The results in numbers
- Average weight loss, including those who stopped treatment: 13.6%, compared with 2.2% with placebo.
- Among those who stayed on treatment: 16.6%, compared with 2.7% with placebo.
- Weight loss of 20% or more: 34.4% of those who stayed on treatment, compared with 2.9% with placebo.
- Early responders: people who lost 10% or more by week 16 reached an average weight loss of 21.6% by week 64.
Put simply, about one in three patients who stayed on treatment lost a fifth of their body weight. The early-responder data were presented at the ECO 2026 congress. Keep in mind, however, that results vary from person to person. Further data appear in the company’s announcement of the study’s publication.
Benefits beyond weight
In addition to weight loss, oral Wegovy improved other health measures. Waist circumference shrank by about 15 cm, and systolic blood pressure fell by about 8 mmHg. Patients also reported better quality of life and physical function.
How to take oral Wegovy correctly
Absorption of the pill is highly sensitive to food and drink, so the way you take it is part of the treatment itself:
- Take one tablet in the morning, on an empty stomach, as soon as you wake up.
- Swallow it with only a little water, up to about 120 ml. Not with coffee, tea or juice.
- Wait at least 30 minutes before eating, drinking or taking other medications.
- Swallow the tablet whole, without splitting or chewing it.
- Do not take more than one tablet a day.
- Missed a dose? Skip it and continue as usual the next day.
Increasing the dose
The medication is taken once a day, starting at 1.5 mg. The dose is then increased gradually every 30 days: 1.5 mg → 4 mg → 9 mg → 25 mg. This lets the body adjust until it reaches the 25 mg once-daily maintenance dose:
- Days 1 to 30: 1.5 mg a day
- Days 31 to 60: 4 mg a day
- Days 61 to 90: 9 mg a day
- Day 91 onward: 25 mg a day, the maintenance dose
If severe side effects occur, your doctor may delay the next increase. If the full dose is not tolerated, the US prescribing information suggests considering a switch to a weekly 1.7 mg injection. Switching the other way, from injection to pill, is covered in my article on switching from injection to pill.
Side effects of oral Wegovy
The side effects are similar to those of the injection, and most involve the digestive system. In OASIS 4, they were reported by 74.0% of patients on the medication, compared with 42.2% on placebo. Nausea occurred in 46.6%, compared with 18.6%. Vomiting occurred in 30.9%, compared with 5.9%.
Even so, most side effects are mild to moderate and fade over time. In practice, only 6.9% stopped treatment because of side effects, compared with 5.9% with placebo. For practical tips, read my guide to Wegovy side effects.
Rare risks
The prescribing information warns of pancreatitis, gallbladder disease and kidney injury due to dehydration. In people with diabetes, retinopathy may worsen, and low blood sugar can occur when the medication is combined with insulin. Before anesthesia or deep sedation, it is important to tell the medical team about the treatment, because of the risk of inhaling stomach contents.
Who should not take the medication
The Wegovy pill must not be taken by anyone with a personal or family history of medullary thyroid cancer or MEN 2 syndrome. It is also unsuitable for people who have had an allergic reaction to semaglutide, and it should not be taken during pregnancy.
Oral Wegovy vs. injections and other pills
- Wegovy injection, 2.4 mg: similar weight loss, with an injection once a week and no fasting rules. A full comparison appears in my article on the Wegovy pill vs. injection.
- Rybelsus: the same active ingredient, at a low dose and for diabetes only.
- Indirect comparison with Foundayo: an indirect analysis of the two drugs’ trials found greater weight loss with the Wegovy pill, by a margin of 3.16 percentage points. Stopping treatment because of side effects was also more common with Foundayo.
- Foundayo (orforglipron): a non-peptide pill, so it can be taken at any time of day, with or without food. In the ATTAIN-1 trial, the 36 mg dose led to an average weight loss of 11.2% after 72 weeks, compared with 2.1% with placebo. For details, see Wegovy pill vs. Foundayo.
Convenience in managing treatment
In terms of managing treatment, the Wegovy pill gives doctors and patients a convenient oral option. It has no common drug interactions, including with birth control pills, so there is no need for additional contraception because of the pill itself.
In short, the Wegovy pill is especially well suited to people who want effectiveness similar to the injection, without a needle. On the other hand, people who struggle with a fixed morning routine may prefer a weekly injection or Foundayo.
Approval status worldwide and in Israel
The US Food and Drug Administration (FDA) approved the medication on December 22, 2025. It launched in the United States in January 2026. According to reports, the out-of-pocket price started at $149 a month for the lower doses. In Europe, the scientific committee of the European Medicines Agency recommended approval in May 2026.
In the United States, the Wegovy pill is also approved to reduce the risk of major cardiovascular events in suitable patients.
The Wegovy pill in Israel
Important to know: at present, the Wegovy pill (semaglutide 25 mg) is not approved in Israel for the treatment of obesity. However, it is available through Form 29C, the Israeli Ministry of Health pathway for importing medications not yet registered in the country.
- Starting doses (1.5 mg and 4 mg): ILS 760.
- Maintenance dose (25 mg): ILS 1,261.
The latest updates appear in my article on the Wegovy pill in Israel.
Practical tips for success with oral Wegovy
- Keep the pill and a glass of water by your bed, so it is the first thing you do in the morning.
- Plan your morning coffee for half an hour after the pill.
- Eat small meals and stop at the first sign of fullness.
- Include protein at every meal and do strength training to preserve muscle mass.
- Drink enough water throughout the day, especially while the dose is being increased.
Frequently asked questions about the Wegovy pill
Is oral Wegovy as effective as the injection?
In clinical trials, weight loss is similar. In practice, effectiveness also depends on following the dosing rules carefully.
Can I take the pill in the evening?
No. Take it in the morning on an empty stomach, and wait half an hour before eating.
How long does treatment last?
Obesity is a chronic disease. So, as with the injections, treatment usually continues over the long term. Stopping often leads to regaining some of the weight, as explained in my article on stopping GLP-1 medication.
Can I switch from the injection to the pill?
Yes, in many cases. The switch follows a plan set by your doctor.
Is the pill suitable for people with diabetes?
The OASIS 4 trial included people without diabetes. For people with diabetes, the decision is therefore made individually, with attention to their other diabetes medications.
The bottom line
Oral Wegovy offers a highly effective oral option for treating obesity. In the trial, those who stayed on treatment lost about 17% of their body weight on average, with a well-established safety and tolerability profile. On the other hand, it requires a precise morning routine. It therefore has the potential to widen the circle of patients who are suitable for medication-based treatment.
