Wegovy pill vs. Foundayo: two new pills in three months
The Wegovy pill vs. Foundayo comparison is new, because the two pills reshaped the landscape of obesity medication in the space of three months. On December 22, 2025, the US Food and Drug Administration (FDA) approved the Wegovy pill, a daily semaglutide tablet that reached US pharmacies in January 2026. Then, on April 1, 2026, the FDA approved Foundayo, a daily orforglipron pill.
Both activate the receptor for GLP-1, a satiety hormone the gut releases after a meal. They differ, however, in their molecular structure, in how patients take them and in how much weight loss they produce. These differences change the clinical decision.
Two pills, two completely different molecules
The Wegovy pill contains semaglutide. It is a peptide, a short chain of amino acids, and it is the same substance found in the weekly injection. The digestive tract breaks down peptides, so only a very small fraction of each dose reaches the bloodstream.
That is why the tablet contains an absorption enhancer called SNAC (salcaprozate sodium), a salt that helps the drug cross the stomach lining. It is also the reason the dosing routine is so strict.
Foundayo, by contrast, contains a small molecule that is not a peptide. The body absorbs it well on its own, so it needs no absorption enhancer. As a result, you can take it at any time of day, with or without food. This one difference explains almost all of the practical differences between the two.
Wegovy pill vs. Foundayo at a glance
- Active ingredient: oral semaglutide in Wegovy, orforglipron in Foundayo. Both activate the GLP-1 receptor, and both are once-daily pills.
- Maximum approved dose: 25 mg a day for Wegovy. For Foundayo, 17.2 mg a day, a tablet equivalent to the 36 mg capsule used in the trials.
- Timing with meals: You take Wegovy in the morning on an empty stomach, with up to 120 ml of water, followed by a wait of at least 30 minutes. By contrast, you can take Foundayo at any time, with or without food.
- Weight loss in the all-participant analysis: 13.6% at 64 weeks with Wegovy and 11.2% at 72 weeks with Foundayo. Assuming full adherence: 16.6% versus 12.4%.
- Size of the main trial: 307 participants for Wegovy, 3,127 for Foundayo.
- Track record and evidence: semaglutide has extensive clinical experience and a US indication to reduce the risk of cardiovascular events. Orforglipron is new and does not yet have a dedicated cardiovascular outcomes trial in obesity.
How much weight do people lose with the Wegovy pill?
The OASIS 4 trial was a randomized, double-blind trial. It included 307 adults without diabetes with a BMI (body mass index) of 30 or higher, or 27 or higher with a related condition. Participants received oral semaglutide 25 mg a day or placebo, together with lifestyle changes.
In the analysis that included all participants, even those who stopped treatment, weight loss after 64 weeks was 13.6% versus 2.2% on placebo. The difference was 11.4 percentage points (95% confidence interval: 9.0 to 13.9). In the analysis assuming full adherence, weight loss was 16.6% versus 2.7%.
How much weight do people lose with Foundayo?
The ATTAIN-1 trial was far larger. It included 3,127 adults with obesity and without diabetes, who received orforglipron 6, 12 or 36 mg a day or placebo for 72 weeks. According to an explanation from Lilly, the 36 mg capsule used in the trial is equivalent to the 17.2 mg tablet approved today.
In the all-participant analysis, weight loss was 11.2% (95% confidence interval: 10.4 to 12.0), compared with 2.1% on placebo. In the analysis assuming full adherence, it was 12.4% versus 0.9%.
At the highest dose, 54.6% of participants lost 10% or more of their body weight, 36.0% lost 15% or more and 18.4% lost 20% or more. On placebo, the rates were 12.9%, 5.9% and 2.8%, respectively.
Wegovy pill vs. Foundayo: which is more effective?
No randomized trial has directly compared the two pills in people with obesity who do not have diabetes. So any comparison of the numbers above is a comparison between separate trials, with different populations, follow-up periods and dropout rates.
The most rigorous comparison available today is a population-adjusted indirect comparison published in 2026 in the journal Diabetes, Obesity and Metabolism. The researchers took the individual patient data from OASIS 4 and the aggregate data from ATTAIN-1, then matched the two by sex, starting weight and blood sugar status.
The result showed an advantage for oral semaglutide 25 mg. In the all-participant analysis, the advantage was 3.2 percentage points (95% confidence interval: 0.4 to 5.9). In the analysis assuming full adherence, it was 3.0 percentage points (0.3 to 5.8). Put simply, both results are statistically significant, but the lower end of the confidence interval is close to zero.
It is important to note that Novo Nordisk, the maker of semaglutide, funded and conducted this comparison. It is therefore supporting evidence, not a substitute for a head-to-head trial.
ACHIEVE-3: a finding that complicates the picture
The ACHIEVE-3 trial is the only direct comparison of the two molecules to date. However, researchers conducted it in people with type 2 diabetes, and it found an advantage for orforglipron.
The trial enrolled 1,698 people, who received orforglipron 12 or 36 mg or oral semaglutide 7 or 14 mg for 52 weeks. With orforglipron, the reduction in HbA1c was greater than with semaglutide. HbA1c is the measure of average blood sugar control over the previous three months. In addition, according to Lilly, weight loss was 9.2% at the highest dose, compared with 5.3% with semaglutide 14 mg.
Still, precision matters here. In this trial, participants received semaglutide at its diabetes doses, 7 and 14 mg, not at the obesity dose of 25 mg. So ACHIEVE-3 does not contradict the indirect comparison. Rather, it shows how much the answer depends on the dose and the population.
Side effects and tolerability
With both drugs, the common side effects involve the digestive system. They include nausea, vomiting, diarrhea, constipation, abdominal pain, bloating and heartburn, and they are most noticeable during dose escalation. I cover this in detail in my guide to the side effects of weight-loss medications.
In OASIS 4, 74.0% of patients on semaglutide reported digestive side effects, compared with 42.2% on placebo. In ATTAIN-1, 5.3% to 10.3% of patients stopped treatment because of a side effect, depending on the dose, compared with 2.7% on placebo.
The indirect comparison found a higher likelihood of stopping treatment with orforglipron. The odds ratio was 4.1 for stopping because of any side effect and 13.9 for stopping because of digestive side effects. The second figure, however, rests on few events, and its confidence interval is very wide: 2.0 to 96.0. In ACHIEVE-3, too, the rate of digestive side effects was higher with orforglipron: 58% to 59%, compared with 37% to 45% with semaglutide.
Finally, both drugs carry a boxed warning in the US. This is the most serious warning on a US drug label, and it concerns thyroid C-cell tumors. The labels also warn about pancreatitis and gallbladder disease.
Wegovy pill vs. Foundayo in everyday life
You must take the Wegovy pill according to a strict routine. Take it in the morning on an empty stomach with no more than 120 ml of water, and swallow the tablet whole. Then wait at least 30 minutes before coffee, food or other medications. According to the dosing guidelines, the dose increases every 30 days: 1.5 mg, 4 mg, 9 mg and finally 25 mg.
On the other hand, you can take Foundayo at any time, with or without food. You also swallow it whole. Its dose escalation, however, takes longer: 0.8, 2.5 and 5.5 mg, followed by 9, 14.5 or 17.2 mg depending on the response, with at least 30 days at each step. In other words, patients reach the maximum dose only after about five months.
Why flexibility is a clinical advantage
In practice, this is the most meaningful difference. Think of a patient who takes levothyroxine in the morning and drinks coffee as soon as they wake up. That person will find it hard to keep to the fasting window that oral semaglutide requires.
Inconsistent absorption means inconsistent results. This is where Foundayo’s flexibility becomes a clinical advantage, not just a convenience. In Israel, Foundayo has been available since mid-September 2026, as I wrote in my article on the pill’s arrival in Israel.
How to choose in practice
- When the goal is the greatest weight loss and a broad evidence base: the Wegovy pill has the edge.
- When the morning fast is likely to undermine adherence: a clear advantage for Foundayo.
- In established cardiovascular disease: semaglutide has a US indication to reduce the risk of cardiovascular events. Orforglipron does not yet have such an indication. However, in the ACHIEVE-4 trial it met its cardiovascular safety endpoint against insulin glargine in people with type 2 diabetes.
- For people who have already lost weight on injections and are looking for an oral maintenance treatment: there are dedicated data on orforglipron for weight maintenance.
- In diabetes, retinopathy, insulin therapy, gastroparesis or critical morning medications: the choice requires individual medical tailoring.
What about Mounjaro? However, you should not compare these numbers with Mounjaro or Zepbound. Foundayo is not an oral version of tirzepatide, and it does not act on the GIP receptor. You’ll find a comparison of the most powerful injections in my article on Mounjaro vs. high-dose Wegovy, and a broad overview of the oral options in my guide to pills for obesity treatment.
The bottom line: Wegovy pill vs. Foundayo and adherence
Comparing the Wegovy pill and Foundayo, Wegovy currently appears slightly stronger in average weight loss. In the indirect comparison, its advantage is about three percentage points. Its active ingredient also has a much broader clinical track record. Foundayo, on the other hand, is far easier to take, and that difference is not trivial. A medication that a patient takes correctly is better than a stronger one taken incorrectly.
So the right question is not only which drug produces the greatest weight loss in a trial. It is which drug the patient will be able to stick with for years. You’ll find another comparison of the two pills in my article on Foundayo vs. oral semaglutide. The European Union approved the Wegovy pill in July 2026, and its launch in Israel should follow later. It is therefore worth checking what is available and what coverage applies at any given time.
