Wegovy for fatty liver: more than weight loss
Wegovy for fatty liver is no longer just an interesting hypothesis. In a large trial, Wegovy (semaglutide) proved that it improves both inflammation and scarring in the liver. As a result, semaglutide received accelerated or conditional approval in the US, Europe and the UK to treat fatty liver inflammation with moderate to advanced scarring, without cirrhosis. This is a real change for millions of people living with obesity.
Fatty liver: the silent disease of the obesity era
Today we call metabolic fatty liver disease MASLD, short for metabolic dysfunction-associated steatotic liver disease. In 2023, a broad international consensus replaced the old name, NAFLD. The goal was to highlight the link to obesity, diabetes and high blood pressure, and to reduce stigma.
This disease is extremely common. According to a global review, about 30% of adults worldwide live with fatty liver. In the Middle East and North Africa, the rate is about 36.5%. In addition, another meta-analysis found that among people with obesity, the rate reaches about 75%. For some of them, the fat triggers inflammation, and then the condition is called MASH, or metabolic dysfunction-associated steatohepatitis. Over time, the inflammation can cause fibrosis, that is, scarring, and later cirrhosis.
What’s more, fatty liver is not only a liver problem. A meta-analysis of about 5.8 million people found a link to a 45% higher risk of cardiovascular events.
The road to the big trial: a phase 2 study
The story began with a phase 2 trial published in NEJM. This trial included 320 patients with fatty liver inflammation, who received a daily semaglutide injection at different doses for 72 weeks. Among the 230 participants with stage F2 to F3 scarring, at the highest dose of 0.4 mg a day, the inflammation resolved in 59%, compared with 17% on placebo. In addition, weight dropped by 13% on average.
However, the improvement in scarring, 43% versus 33%, did not reach statistical significance. So the researchers designed a larger, longer trial using the weekly Wegovy dose. That is how the ESSENCE trial was born.
The ESSENCE trial: what researchers tested
ESSENCE is a double-blind, placebo-controlled phase 3 trial. It enrolled 1,197 patients with biopsy-confirmed MASH and moderate to advanced fibrosis (F2 or F3). Participants received a weekly injection of semaglutide 2.4 mg or placebo, in a 2:1 ratio. Everyone also received guidance on diet and physical activity.
The full trial will last 240 weeks. Even so, the results published in NEJM come from a planned interim analysis. This analysis covered the first 800 participants after 72 weeks, with a repeat liver biopsy.
The results: Wegovy for advanced fatty liver
The findings are clear. First, liver inflammation resolved without worsening of scarring in 62.9% of semaglutide patients, compared with 34.3% on placebo. In addition, scarring improved without worsening of inflammation in 36.8%, compared with 22.4%. Both outcomes occurred together in 32.7%, compared with only 16.1%.
Put simply, out of every 100 patients, about 63 saw their inflammation resolve with Wegovy, compared with about 34 without treatment. Meanwhile, weight dropped by 10.5% on average, compared with 2.0% on placebo. Noninvasive liver tests, such as liver stiffness measurement, also improved.
What this means for physicians
Clinically, the trial defined two primary histological outcomes, and both reached significance (P<0.001). The difference in MASH resolution was 28.7 percentage points, and the difference in fibrosis improvement was 14.4 percentage points. Keep in mind that these are surrogate outcomes. Therefore, the second part of the trial will examine clinical liver events, such as cirrhosis, and should end around 2029.
Moreover, the joint European guidelines on liver disease, diabetes and obesity recommend considering incretin-based drugs in patients with fatty liver and obesity or diabetes, when there is an indication. The ESSENCE data add histological evidence of benefit in the liver itself.
Does Wegovy help a fatty liver only through weight loss?
Weight loss is a proven treatment for fatty liver. In other words, when weight goes down, less fat reaches the liver and insulin sensitivity improves. Still, that may not be the whole story.
A new study in mice, published in 2026, found GLP-1 receptors on the cells of the tiny blood vessels in the liver. There, semaglutide improved the liver even in mice that did not lose weight. On the other hand, when the researchers removed the receptor only from the liver’s blood vessel cells, most of the liver protection disappeared, even though the mice kept losing weight. These are animal data only. Nevertheless, they hint at a direct effect of the drug on the liver.
What real-world data show
Real-world evidence, meaning data from routine care outside clinical trials, adds an important angle. For example, a multicenter study in patients with diabetes and fatty liver compared semaglutide with other diabetes drugs. The risk of serious liver events was about 27% lower compared with SGLT2 inhibitors.
In addition, a target trial emulation study found a 19% lower risk of serious liver events with semaglutide, compared with a DPP-4 inhibitor. In a large study from the US Veterans Health Administration, researchers found a link between GLP-1 drugs and a lower risk of cirrhosis. But in people who already had cirrhosis, the researchers found no such link. This finding supports the importance of early diagnosis and treatment.
Note that these are observational studies, all in people with type 2 diabetes. So they show an association, not necessarily cause and effect.
Safety and side effects
Side effects in ESSENCE were mainly digestive. Nausea occurred in 36.3% of semaglutide patients, compared with 13.2% on placebo. Diarrhea, constipation and vomiting were also more frequent than on placebo. However, only 2.6% stopped treatment because of side effects, compared with 3.3% on placebo. The researchers did not identify any new safety signals. In the phase 2 trial, slightly more tumors occurred in the semaglutide group, with no pattern pointing to a particular organ, and doctors continue to monitor this issue.
Side effects appear mainly while the dose is going up. That is why a gradual increase, small meals and enough fluids help a great deal. Read more in my nutrition guide for weight loss injections.
Who is Wegovy for fatty liver suitable for?
The FDA approved Wegovy in August 2025 for adults with MASH and stage F2 to F3 fibrosis, without cirrhosis. In the European Union, semaglutide received conditional approval for this indication in March 2026, under the brand name Kayshild. In the US and the UK, the liver indication applies to Wegovy 2.4 mg. All of these approvals depend on clinical results from the second part of the trial. Ozempic, which is intended for diabetes, is not approved for this indication.
So the first step is a proper evaluation. The FIB-4 score, an index calculated from routine blood tests, and a FibroScan both estimate the degree of scarring. Your treating physician will always make the treatment decision with you, based on your personal situation.
Wegovy for fatty liver: what you can do today
Alongside medication, lifestyle remains the foundation. According to professional guidelines, losing 7% to 10% of your weight significantly improves the liver. A Mediterranean diet, less sugar and fewer sweetened drinks, regular physical activity and avoiding alcohol also help. Questions and answers about Wegovy for fatty liver
Does Wegovy cure fatty liver?
In ESSENCE, liver inflammation resolved in about 63% of patients, compared with 34% on placebo. Scarring improved in about 37%, compared with 22%. This is a significant improvement, but not a guaranteed cure for everyone. In addition, there are still no data on what happens to the liver after stopping treatment, and ongoing treatment is likely to be necessary.
Is the treatment suitable for cirrhosis?
No. Current approvals cover disease without cirrhosis, with stage F2 to F3 fibrosis. In a large observational study, researchers found no link to benefit in people who already had cirrhosis. That is why early diagnosis and treatment matter.
How long does it take to see improvement?
In ESSENCE, researchers examined the liver after 72 weeks, or about a year and four months. However, weight loss and better blood test results usually appear earlier. Along the way, we monitor progress with blood tests and noninvasive scans.
Is diet alone enough?
Significant weight loss improves the liver, and it remains the foundation of any treatment. On the other hand, most people find it hard to maintain that kind of weight loss over time. This is where medication can help, always alongside lifestyle changes.
The bottom line
Wegovy for advanced fatty liver now rests on a large phase 3 trial. The drug resolved liver inflammation in almost two-thirds of patients and improved scarring in more than a third. Real-world data strengthen the picture, but we are still waiting for long-term results. If you live with obesity and fatty liver, it is worth checking the state of your liver and discussing the options with your doctor.
